The Iowa Department of Inspections and Appeals conducts an unannounced, annual survey of each skilled nursing community in Iowa. Surveyors conduct their survey for several days, and they review over 500 quality, safety and performance measures. They interview staff, observe care, and check medical records for compliance to regulations on care, confidentiality, cleanliness, residents’ rights, and quality of services. The survey includes a community tour, review of residents’ clinical records, and interviews with residents and family members about life and the care they receive.

Since the survey is unannounced, our team must consistently offer the highest quality of work and care.

Kennybrook Village was awarded the AHCA Quality Initiative Award by achieving multiple AHCA goals. Only 63 out of 450 senior living communities in Iowa were given this award.

According to, the Quality Initiative Recognition Program recognizes senior living communities that achieve at least four of the eight goals the AHCA has set. Listed below are the goals Kennybrook Village achieved:

  1. Adopt Core-Q customer satisfaction questionnaire and measure and upload data to LTC Trend Tracker.
  2. Safely reduce the number of hospital readmissions within 30 days during a skilled nursing center stay by a total of 30% from December 2011 or achieve and maintain a low rate of 10%.
  3. Improve discharge back to the community by 10% from December 2014 or achieve and maintain a high rate of at least 70%
  4. Improve functional outcomes by 10% from December 2015 or maintain an average rate of improvement of 75%

As a Medicare-certified care center, Kennybrook Village undergoes a rigorous onsite inspection survey by CMS staff each year. A CMS Five-Star quality rating ranks Kennybrook Village in the top 10 percent of retirement communities in the state.

CMS created the Five-Star quality rating system to help individuals, families and caregivers compare retirement communities and rehabilitation centers more easily. The Five-Star rating system is based on information from annual state health inspections, staffing ratios, and a range of different quality measures such as cleanliness, staff responsiveness, rehospitalization, infections, and pain management. Scores in each of these three individual areas are then used to determine the overall star rating.
